POLICE hold grave concerns for a four-month-old boy who went missing with his father in Perth four days ago.

Zach O'Kane's father telephoned the family home on Wednesday and said he would be back that night, but never showed, major crime squad officers say.

That was two days after the mother of the child last saw her son when her de facto partner John Patrick O'Kane, an unemployed truck driver, said he was taking Zach to visit his family in Langford, in Perth's south.

Police doubt whether the baby was taken there.

"Mr O'Kane had taken Zach to the family member's house on one previous occasion but it wasn't an overnight stay ... the de facto was quite satisfied with allowing Zach to go away for two nights," Detective Senior Sergeant Mark Fyfe said.

"So now that he hasn't been seen since Monday we are holding grave concerns for Zach's welfare.
"We have concerns that Zach never arrived at the family member's house in Langford but we're unable to actually confirm whether he did or did not arrive.

"There was a baby's bag taken (containing) formula, bottles and a small amount of clothing, (but) there wasn't enough formula to last past yesterday."

Police say there had been no reported incidents of domestic violence between Mr O'Kane and his partner and the relationship appeared to be a stable one.

Mr O'Kane had lived in various states in previous years but police would not say whether he was previously known to them.

He is believed to be driving a light-blue 1992 Suzuki Swift hatchback, registration 8JG 690.
